DESCRIPTION:Goals of care conversations which are different from but connected to advance care planning. \n\n\n\nGoals of care conversations are discussions LTC Staff have with people living with a life-limiting illness about their goals\, values and beliefs so that LTC can align the care provided with what matters most to the person.
